Monthly Maintenance fee is based on a single or a family rate for the amount of accidental death and dismemberment coverage.

Single Plan           Maintenance Fee

If this is a joint account, the amount of insurance would be $10,000 per person.

Family Plan           Maintenance Fee

$2,500 worth of coverage per dependent
